STEPS AND REQUIREMENTS:
A. Local


1. Applicant to Private Higher Education Institutions(PHEI)/ Local Universities and Colleges (LUC)
The applicant submits the original and photocopy requirements for the application of CAV;
  • Diploma (for Graduate)
  • Official Transcript of Records (TOR)
  • Certification of the total no. of units earned (for Unit Earner)
Others (if applicable):
  • Certification of Correction of Name
  • Notarized authorization for applicant through a representative with Valid IDs of both parties

2. Private Higher Education Institution(PHEI)/Local Universities and Colleges (LUC) to CHED
The Registrar certifies the authenticity of the documents and endorses it to CHEDRO (Regional Office).
  • Prescribed Endorsement Form
  • Diploma (for Graduate)
  • Official Transcript of Records (TOR)
  • Certification of the total no. of units earned (for Unit Earner)
  • Proof of Payment

Others (if applicable):
  • Certification of Correction of Name
  • Notarized authorization for applicant through a representative with Valid IDs of both parties

3. CHEDRO (Regional Office)
The Records Office validates the completeness of the requirements;
  • If complete: Records Office forwards the CAV application to cashier for Official Receipt.
  • If payment is confirmed: Cashier issues Official Receipt and forward the same to Records Office.
  • Records Office process the CAV.
  • If approved: Records Office releases the CAV in Security Paper to PHEI/LUC.
  • If with deficiency: Records Office returns the application to PHEI/LUC concerned.

B. For Abroad
For CAV application to be used outside the Philippines, follow Steps 1, 2, and 3 provided in A. Local.
The applicant brings the CAV in sealed envelope to the preferred DFA Branch/Office. See the list below.​
In the case of applicants from STATE UNIVERSITIES AND COLLEGES (SUCs), read footnote at the bottom part of this thread.​

The DFA will no longer issue Authentication Certificates with "red ribbon", and will instead affix an Apostille as proof of authentication to documents for use abroad.

Apostille - is a certificate that authenticates a public document's origin and is a requirement for all Philippine documents to be used in countries party to the Apostille Convention. With an apostille, there's no need for authentication from foreign embassies or consulates in the Filipino traveler's destination.

C. Individual filing of CAV for Closed HEI.
The applicant submits the original and photocopy requirements for the application of CAV;​
  • Notarized Request letter
  • Diploma (for Graduate)
  • Official Transcript of Records (TOR)
  • Certification of the total no. of units earned (for Unit Earner)
  • Proof of Payment (Fund Transfer/Validated Bank Deposit Slip)
Others (if applicable):​
  • Certification of Correction of Name
  • Notarized authorization for applicant through a representative with Valid IDs of both parties.
    • Through Courier (Preferably JRS with Return Prepaid Pouch)
D. Certified True Copy (CTC)
The applicant may apply CTC of CAV Security Paper together with the CAV application to PHEI/LUC;​
For CTC, only individual can directly request CTC to CHEDRO (Regional Office) and presents the following:​
  • Original CAV
  • Proof of Payment
Payment Details:
Per CHED Administrative Order (CAO) No.04 s.2014​
  • CAV – ₱ 80.00
  • CTC – ₱ 30.00 per copy

Note: In the case of applicants from STATE UNIVERSITIES AND COLLEGES (SUCs) and other public higher education institutions created by special laws; issuance of Certification, Authentication and Verification (CAV) is now left to the concerned Higher Education Institution (HEI) and no longer requires authentication by CHED.​
