Guys, meron na po ba sa inyong nakapag pa renew ng passport kasi nawawala ang passport nyo? Ang issue kasi sa pag papaappoint sa DFA online application kapag mag rerenew ka may hinihingi pang OLD Passport #/Issued date/ at kung sang DFA. sa tatay ko kasi yung irerenew ko at 2001 pa yung passport nawawala. Anong nilagay nyo dun sa OLD Passport #/Issued date/ at kung sang DFA? Sana po ay may makatulong. Nag email nako sa DFA pero need ko malaman agad kung meron dito para makapag paappoint agad.
 
Nakapag renew ako ng passport 2015 pa walkin lang ako wala pang online. Kelangan nila yung old passport kasi bubutasan nila yun. If ever lost I think kelangan mo ng affidavit of loss.

Sa page 1 or 2 ng passport makikita mo yan doon lahat nakalagay

Birth Certificate (BC) in Security Paper (SECPA) issued by the National Statistics Office (NSO) or Certified True Copy (CTC) of BC issued by the Local Civil Registrar duly authenticated by NSO depending on the NSO signatory; for born 1950 & below, Negative Birth Record from NSO & Joint Birth Affidavit from Two Disinterested Persons if no record from NSO.

Call (02) 737-1111 for your NSO Birth Certificate

If Birth Certificate (BC) if late registered:

  • If registered on or before 2000: submit 1 original supporting document indicating full name, date and place of birth, and citizenship prior to the date of late registration.
  • If registered on or after 2001: submit 2 original supporting documents indicating full name, date and place of birth, and citizenship prior to the date of late registration.
Original Valid ID

Original ID i.e. Senior Citizen’s ID, Voter’s ID, digitized government-issued IDs like SSS, PRC, BIR, Driver’s License, original school ID (for students only) AND original supporting documents indicating full name, date and place of birth and citizenship.


Other Supporting Documents

Please bring the following if they are available.

  • Marriage Contract
  • Land Title
  • Driver’s License
  • Government Service Record
  • School Form 137 or Transcript of Records with dry seal
  • Other documents that show full name and birth details of applicant and/or citizenship
  • Voter’s Registration Record from COMELEC Intramuros
  • Baptismal Certificate with dry seal
  • Seaman’s Book
  • Income Tax Return (Old)
  • NBI Clearance

yep, kaso nga sa online system may part na need fillupan kung renew ang gagawin. Di ko alam kung dapat ba renew or new application gagawin ko.

Mahirap na ngayon kasi online na at pahirapan makakuha ng appointment. Kung Malapit lang kayo sa Dfa branch pwedi kang pumunta doon meron naman silang assisstance desk doon. Notarized affidavit of loss lang naman usually hinihingi kapag nawala. I think hindi mo na kelangan mag Online in that case try mo pumunta sa dfa.

Kaya nga paps eh, napakahirap mag hanap ng bakanteng slot. talagang binabantayan ko pa may ma open. May malapit namang branch ang problema 6pm na kasi out ko kaya sa malamang sarado na yun. Nakapag panotaryo na naman kami.
