Help Birth certificate surname problem

Hello po mga lodi may issue lang po ako sa birth certificate ko. Hindi kasi kasal magulang ko tapos nakatira ako sa tatay ko. Bale ang nasa birth certificate ko is apelyido ng mama ko pero gusto ko sana dalhin apelyido ng tatay ko. Paano po kayo ang proseso para maayos ito and magkano kaya magagastos? Maraming salamat po

your lawyer will file a case requesting the said revisions. the judge will make a decision whether to approve the request or not, based on the hearing when you are asked to prove that you are that same person on the birth cert and also to prove why you need the revision.

barring any obstacles the judge will render a decision ordering the PSA to make the requested revisions.

Kung ipinanganak ka after August 3, 1988, mga affidavit na galing sa tatay mo na nagsasabing anak ka nga niya at affidavit nang pagpayag niya na gamitin mo ang apelyido niya ang ipapasa mo sa local civil registrar kung saan ka ipananganak. Ipasa mo rin lahat ng iba pang relevant documents. Yung bayad aabutin ng ilang libo depende sa singil ng local civil registrar at aabutin ng ilang buwan yung proseso. Updated na yung batas diyan. Basta yung mga obvious na clerical errors sa birth certificate, ipa-file na lang sa local civil registrar mga corrections.

pa-assess mo lang fees mo sa LCR kasi sila magbibigay ng mga documentary requirements mo at ang attorney is hindi libre
